Registration for Players &Volunteers

Visit www.eayso.org and follow the instructions to register your child. Please print out 2 copies of the registration form, sign and initial both copies, and mail in with a copy of child's birth certificate.

 BOTH FORMS MUST BE SIGNED BY THE PARENT/GUARDIAN OF THE CHILD.

Online Registration $85.00

Payments that are mailed in may be made by check, or money order.

Credit card payments are now accepted online (once logged into eayso, click on the Player Online Payment button and follow the instructions)

For Boys and Girls ages 4 to 15 (must turn 4 by July 31, 2011)

 

Registration Fee includes:

*  Full uniform to keep (jersey, shorts, and socks)

*  Memory Mate Photo Package

*  AYSO Soccer Accident Insurance plan (covers injuries during practices & games play)

* 10 to 12 weeks of play depending on the age

Mail-in registrations are currently being accepted. Registrations received after June 12th will not be dropped onto teams until July 24th, 2010. They will be placed on a wait list to make certain there are coaches available to coach additional teams. Mail-in registrations must provide the following,

1.                  Two copies of the registration form completely filled out, signed, and initialed by a parent/guardian. (If forms are not signed or initialed, they will be returned)

2.                  A copy of the players birth certificate. (New players only)

3.                  Check or Money Order for registration fee. (Unless paid online using credit card)

If any of these items are missing, we will mail the contents back to the return address.

 

Practice fields are throughout the North Valleys, all games are expected to be held at the North Valley’s Regional Sports Complex.

 

Refund Policy:

While we recognize that plans change and that it is difficult to know what you will be doing months in advance, we cannot always provide a full refund if you decide that your child cannot participate in the Fall season after you have registered.  There are certain unrecoverable costs that AYSO incurs at various times after you register.

To request a refund you must submit the following information in writing by e-mail (treasurer@ayso710.org), letter, or postcard and mail to the AYSO Registrar/Treasurer, 216 Lemmon Dr. #363, Reno, NV 89506.  Please note that a refund request will take a minimum of two weeks and up to 6 weeks (the region must wait for verification from AYSO National that check has cleared prior to issuing the refund) to process.

Refunds are issued as follows:

*    Requests postmarked by July 31st  2011 – Full Refund.

*    Requests postmarked by August 31st – 50% refund with uniform return (to coach please).

*    No refunds will be granted after August 31st, 2011. (No Exceptions)
